This position is responsible for:
Assisting clients with personal care, including dressing, mobility, administering medication, personal hygiene, eating and transporting to doctor’s appointments
Monitor patient medications and update health charts
Maintain open communication between families and health care professionals regarding client’s medical and emotional condition
Document and report any changes in client’s health status
Ensure highest client safety and well-being
Must have skills knowledge to:
Provide treatment that promotes resident dignity, independence, individuality, strengths, privacy and choices. Recognize obvious symptoms of a mental health disorder, personality or substance abuse; Assisting residents in performing activities of daily living
Provides laundry services for residents clothes & linen
Prepares resident meals
Provides complete documentation of ADLs, medications, vital signs, resident activities, intake / output, incident reports, progress notes and any other documents needed to comply
Provide Assistance in the Self-Administration of Medication
Meet the unique needs of the resident population served by the agency or staff member.
Recognize and respect cultural differences.
Recognize, prevent, and respond to a situation in which a resident maybe in danger to self- or a danger to others, behaves in an aggressive or destructive manner, may be experiencing a crisis situation or may be experiencing a medical emergency.
Read and implement a resident's treatment plan
Assist a resident in accessing community services and resources
Record and document resident information.
Identify types of medications commonly prescribed for mental,
personality disorders, and substance and the common side effect and
adverse reactions of the medications.
